Nursing

The Future of health care is Home Care.

The Nurse Practitioner provides medical services including, but not limited to, assessment, treatment, recertification and health teaching in the form of patient home visits. Some responsibilities include:

  • Consult with the patient’s attending physicians, as needed, regarding palliation and management of terminal illnesses; in addition, meet the general medical needs of the patients to the extent that these needs are not met by the attending physician.
  • Conduct face to face encounters for assigned patients entering their third or subsequent benefit period, document findings of encounter, and provide findings to the certifying physician in a timely manner. 
  • Act as a resource to staff who identify patient issues relating to specific clinical areas by recommending a plan of care and implementing when appropriate with the case manager and/or interdisciplinary team.
  • Prescribe and review appropriate medication for therapeutic effectiveness of the patients. Include both drug and non-drug-based treatment methods into the plan of care under the direction of the Medical Director.

This full-time position is based at our Brown Deer headquarters but also travels throughout Milwaukee, Waukesha, Ozaukee, and Washington Counties to visit home care or hospice patients.

Qualifications include a current State of Wisconsin APNP license with prescriptive privledges, plus at least 2 years of home care or hospice clinical experience.

Make the difference of a lifetime.

Hospice nursing is not an easy job, but it is immensely important. Unlike high-volume hospitals and clinics, hospice nurses really get to know their patients and can become an essential part of the family, sometimes for only a short period and other times for many months.

During that critical period near the end of life, a hospice nurse is the person patients turn to for care and families turn to for comfort and education. Little by little, your efforts can have an amazing impact on the lives of these people. It is a meaningful responsibility and, for many, a calling.  There is no job more rewarding.

Our Hospice Case Managers perform referral evaluations / admission assessments of patients and families seeking Hospice services.  Work a desirable Mon-Fri 1st shift schedule while assuming 24-hour accountability for safe, effective nursing care of assigned patient / family caseload through the utilization of the nursing process, clinical skill, and Interdisciplinary Group approach. 

Qualifications include a degree from an Accredited School of Nursing, WI RN license. Previous Nursing experience strongly preferred. Hospice or Oncology experience ideal.

It is particularly important that the Administrative Assistant be an expert Excel user, able to create complex spreadsheets with charts/graphs. Ability to type 45+ words per minute.

Horizon is leading the community in providing a full spectrum of hospice services for both patients and their families, as well as reaching out to the community at large.

 

Horizon is proud to sponsor a comprehensive Grief Resource Center, located at our Brown Deer headquarters. The Center is a safe haven for individuals seeking counseling, support and guidance. This part-time (20 hrs/wk) position will facilitate bereavement services, including assessment, counseling, support groups, adult education, and community outreach. This is wonderful opportunity for a compassionate, professional, experienced counselor to make a difference to people in need.

 

Qualifications include a Master’s Degree in Counseling or related health care or human services field.  Must be a Licensed Professional Counselor in the State of Wisconsin. Minimum 1-2 years experience in bereavement field including counseling, support programming and adult bereavement education.

Full-time position available at our busy Brown Deer office! Assist our Intake Manager with day to day operations of the Intake team. Unique opportunity for compensation if you are currently caring for a loved one.

 

For consideration, you must have cared for this family member/friend for at least 6 months (full-time) or 1 year (part-time) OR have equivalent experience as a PCW/HHA in the medical field. You are not eligible if you are the legal guardian of a patient under the age of 18. Your patient also must have or be eligible for Title 19.

 

Sometimes taking care of a family member can be time-consuming. Why not be employed by Horizon on a part-time basis as a Personal Care Worker and be paid for the work you are already doing?
