
Thank you for your interest in supporting Horizon Home Care & Hospice financially. We are a 501(c)3 not-for-profit organization (Tax ID number 391171298) and many of our programs can only survive through the benevolence of donors like you.
As a not-for-profit organization, we strive to provide the best care to all of those in need. We recieve government and private insurance reimbursement for services provided, and donations are an essential suppliment to these funds. Programs like charity care, Hospice care bags, the Grief Resource Center, the Working Poor program, Hospice special needs fund and more would not be available without generous funding from individuals and organizations throughout this community.
You may choose to identify which area of the agency you'd like your donation to support: Hospice, Home Care or the Grief Resource Center. At certain times of the year, there will be special appeals (listed here) that you may also be interested in joining.
Memorial gifts are also a wonderful way to show your compassion and sympathy while engaging with a wonderful cause. When a memorial gift is made in honor of a Horizon patient, you will recieve an acknowledgement for your gift, and the family of the honored individual will also be notified and provided your name and address. We provide opportunities to create a visual memorial with our Grief Resource Center Memorial Monument and our Hospice Memorial Tree.
